I think you really pulled off a smooth, sophisticated design. It is a sharp, simple presentation. The font you used reinforces this very well; it is tried-and-true in a design like this.
The manip work is restrained and approaching seemless. Before looking at the stock, I could only tell for sure that the darkening of the dark areas (veins), and the top-left corner were post process. The way you cleaned up the "dimples" under the eyes is simply stunning.
The top-left corner's muddy orange-red " charlie brown" lines could stand out less; I don't see any clear advantage to them in the first place. Also, the white cracks could have less contrast -- just a little bit, because they add volume to the shape. With a smooth presentation like this, the teeth could have been more discernable ... although I imagine their vagueness adds age.
The dark lines on the skull remind me of veins; but they do not look photographic. If you are willing to part with photorealism, you could bring them out more; it is an interesting allusion.
